Problem type: Design is broken
Description: The radio buttons are distorted
Steps to Reproduce:

1. Navigate to: http://m.hclips.com/form.html
2. Expand any section.
3. Tap on a Radio button.

Expected Behavior:
The radio button is correctly displayed.

Actual Behavior:
The radio button is incorrectly displayed.

Notes:

1. The issue is also reproducible on Firefox 54.0 Release (Mobile).
2. The issue is not reproducible on Chrome (Mobile) 59.0.3071.125. The radio buttons are checkboxes.
